Often this is a result of people who don’t seek behavioral therapy and drug intervention early on. Or are misdiagnosed and treated irrelevantly. Or don’t comply with prescribed treatment program(s). Chronic PD behaviors are also known to lead to homelessness. Especially when the individual is drug addicted and believes self-medication
